KODIAK INTAKE & SET UP PROCESS

Kodiak is a Clinical Trial client that will have CT locations listed as sub clients under them similar to CRIO & Advarra, except this is NOT an API. We will not be talking to the sub clients ONLY Kodiak.

Flow:
· The sub client request will come in through Salesforce but does not need to go through Due Diligence as all funding will come from Kodiak.
· Sales needs to provide the sub client name and address.
· Once assigned to IM, go through the following process:
Client Set-up:

· Create the Client ID for the sub client under Kodiak Sciences Inc: 3100407
· All sub clients VFA will be Kodiak’s VFA: 3100408
TPA
· Enter the new sub client ID under Kodiak in Pathward.
Program Set-up
· To set up a new sub-climate under Kodiak, follow the following steps:
o Click “Add New Customer”
o Fill in the required information:
o Click next, next and fill out the required information:
o Corporate Payments
o Mastercard- CT2 Lap IIC
o Add Client ID and choose CT2 Generic Mastercard – 18889 Digital package
o Virtual Funding Account: Kodiak Sciences VTA PT Virtual
o Sub Program ID – PERM: 334574
o Choose Package ID – PERM: Kodiak Sciences MC CT2 LAP PW – 21075
o At the bottom choose EOY 1099 Report
o Next
o Invoicing:
§ Card Fee: $3.00
§ Invoicing should go to Kodiak Sciences Inc to their address and not the sub client.

· Campaign Configuration
o Follow the campaign configuration directions in Confluence:

· Admin Users
o MariAnn will provide the admin users for the sub client. They should have the following access:
